No insert, just an open fireplace.
Oh, I thought he started the post about an insert........... If no insert, then definitely a solid block off of some sort. Even cement board would work and be 'workable'.
 
my three cents, once you put a pellet stove in and use it a few times you will be hooked on it and it will become your primary source of heat. one of the best investments I have ever done. good luck with your decision and you get what you pay for
