I need to find a replacement for this fan. Any ideas? Dimensions are 3 inches deep x 5 inches high x 28 inches wide.

Looks like it. If so, just replace with new ones from Grainger.

What insert is this for?
Are all of the fans not working or just some. If all are not working, the problem might be elsewhere like the power switch or thermostatic snap switch.

Not the best photos. Might have to go back and get a few more. I was hoping to avoid pulling the insert to find out the make and model, which is not apparent on the front side.

It may be possible to find a blower unit of 3 fans, but you’d need to find the manufacturer and see if they are still business. Given the patina, and style, it looks like an older unit. You might have a challenge. Post some pics of the whole unit, somebody might be able to identify it.

It looks to me like a sheet metal cover with fans screwed to the back. Many pancake fans come with a clip that plugs into the fan that has color coded wires. It wouldn’t be hard to attach to a new lead that would give you new wiring all the way to the plug. If you do not feel comfortable doing this, a handyman/hearth shop/chimney sweep/electrician could certainly help. Getting a sweep or hearth shop in might also get your unit inspected. Many older inserts were installed in ways that weren’t the safest.

Post a full frontal picture of the insert. Maybe we can ID it from that.

Thanks. I have an electrician friend taking a look at the wiring for me.

That's almost an antique, probably close to 50 yrs old. The odds of getting a new replacement for this are close to zero. It will have to be rebuilt or something substituted.

If this is a slammer install with no liner connected to the insert, it is also no longer code accepted.
