I've been told this is a "Heatilator", but I can't find any marks on it - and I know that it's very incomplete. Any pointers on how to research further what this might be, and if it's worth finding the parts to put it back together? The house was built in 1972.

It's a heat form fireplace meant to be completely enclosed in a masonry structure. As long as the damper is there and the box isn't rusted out you have the parts. But the surrounding area looks very suspect. That all needs to be masonry
 
There is definitely rust, and weirdly there is insulation poking out of the top right. The rest is all masonry (concrete blocks, as far as I can tell). The surround is all Durock, with metal framing (done recently). I'm thinking, since so little is known, that it's a rip/repair/replace job. Thanks!
