I am a bit dubious about the thought of it heating the basement too during very cold weather, or is that just to keep pipes from freezing there?

Agree. If my un-insulated. Un-heated basement never drops below 50 minimum where I live, I can hardly believe it would be a issue there. A bit of electric heat in your basement would be my recommendation.

For good circulation pull air from the far br area and use that to supply air to the fireplace convection blower.
This is doubly important if the intent is to duct thru the attic. Warm air will replace the air being pulled from the farther reaches of the house and the duct losses from running through the cold attic will be less noticeable as this return air gets warmed by the fireplace. It will also help set up a better convection flow throughout the house.

I’m still looking at the 44 Elite. Could what you’re suggesting be done with that unit? I’m just having a hard time imagining how it would all be duct.

So I’d have vents in the far bedrooms, and those vents would be duct back to the fireplace through the attic? The convection (and the Posi-Pressure of the 44 Elite) would pull air through the house?

Could the fan/blower on the 44 Elite be in another room and piped to the fireplace?

Something else I’ve been wondering. On the 44 Elite, how is the Posi-Pressure different than any other fireplace with a blower? It seems most do have some type of fan...
