Can you pull the existing blower out and post a pic? We might be able to match it with an aftermarket unit that will work.

That's a bit unusual to have the two small outlets on a common shaft like this. However, this place has advertized parts for EZ inserts. If no luck, take it to an electric motor shop for a rebuild.
