Question on parts needed for selkirk supervent install.

I'm going from a main floor into a living area that was an attic at one point and then through an 11/12 roof. I know I need to enclose the chimney on the second floor. But I'm having trouble getting a straight answer from the instructions on what kind of shielding I need. Since it is enclosed below the roof I need the Rafter Radiation shield for the roof. Now for the ceiling/floor I'm confused if I need the Firestop Rad Shield/Joist shield and the decorated ceiling support. Does the support fit with the joist shield? Figure 10 shows no Joist shield needed so I think that's correct but i have true dimensional 2x10 joists at that level. Will the rafters be protected by the framing and the ceiling support? Or should I try the joist shield as well?

Bonus questions. I have a cathedral ceiling support. Can I just completely nix the Rafter shield in the roof for the CCS since it allows pass through of the chimney pipe? Would the CCS being enclosed be an issue? I kind of like the idea of using the insulation in the CCS to prevent cold air from seeping in. As for the enclosure, should I insulate it or should I use the Insulated chase plug?
