Need Help Identifying Timberline Fireplace Insert Model Number

I'm hoping someone might be able to help me identify the model number of this Timberline Insert. I need to determine if it's EPA certified.

This stove is definitely not EPA certified and was built prior to EPA phase I regulation. It also may be a slammer installation (no direct flue connection) which is no longer permissible.
