Identify my Fireplace Please

Hello,
Can someone please try and identify the make/model of my fireplace. I have looked everywhere for the placard to no avail.I have also done several image searches and cannot find anything resembling this. It is located in Wyoming and is in a house that was built in 1980 if any of that helps.Thank you!
