The chimney needs a support brace at the 5' point. The DuraVent installation guide shows how and the proper angles. There is a transition adapter that you need to have to connect the DVL to the support box.

Sooteater from the bottom up is fine. That's the way I clean our chimney. Make sure the cap screen gets clean if there is one.

5" from the roof line as measured from the top or the bottom where the flue meets the roof? 5" is better than 4'.

There is a transition adapter pictured on the box to the chimney kit but mine didn't have the ring. That is ok,
I'm tired and this gives me a break.
 
5' up from the exit point at the roof.
